COMPANY VEHICLE PERSONAL USE REQUEST

Important Company Vehicle Usage Policy

In order to streamline operations and ensure responsible use of company resources, we have implemented the following policy regarding the personal use of company vehicles:

Approval Requirement: All personal use of company vehicles must be pre-approved by management. You are required to submit a formal request on this form for approval. Notification of either approval or denial will be emailed to you on the day you make your request.

Mileage Charges: Mileage exceeding 100 miles within a 6-month period will incur charges. The charge will be calculated at a rate of $0.17 below the published IRS rate at the time of the request (current rate is $0.67/mile). This charge will commence after the 100-mile mark within that period.

Emergency Use: Emergency use of company vehicles will be evaluated on a case-by-case basis. You are required to explain the nature of the emergency to management for a approval. No fees will apply for approved emergency use.

Important Vehicle Maintenance Notification Policy

Please fill out this form whenever you bring your vehicle in for maintenance and repair work. It’s important to make sure that the vehicle you’re responsible for doesn’t miss any scheduled maintenance appointments. This helps keep the warranty and repair policies for the vehicle valid and active.

In the EMS Employee Handbook, you’ll find details about your responsibilities when it comes to driving a company vehicle. Generally, we schedule tune-ups, oil changes, lubrication, fluid top-ups, tire rotations/changes, alignments, and filter changes in the spring (April) and fall (October).

Please complete this form with details of the maintenance and/or repair work needed for your vehicle before taking it to the dealer.
